Yep, that's the one. They didn't understand how the reaction would happen and didn't understand the fallout implications and that was after 10 years of messing with the devices. It makes no sense to think their understanding was better for the 2nd and 3rd bombs ever detonated, especially since they were different designs using different nuclear material.

At this rate, the Japanese will probably just develop natural protection against radiation.

Now, if only someone can prevent the spread of Nancy Grace...
